New Patient Registration
Please bring your medical card to reception. If you cannot find your medical card please inform our reception staff who will provide the appropriate form for completion. If you are a non UK resident you will also need to bring your passport/personal identification card (PIC) and visa/work permit.
Where patients are requesting to join the practice list, the practice does not discriminate on the grounds of race gender, social class, religion, sexual orientation, appearance, disability or medical condition.
All new patients are offered a health check questionnaire to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
Medical treatment is available from the date of registration. Please contact reception for further information.
Practice Boundary
The practice area covers a 9 mile radius of Tynan including Tynan, Killylea, Caledon, Middletown, Aughnacloy etc.
Change of Address/Details
It is imperative that we have your correct details at all times. If you change your name, address or telephone number please let us know.
